Metal Ion Levels in Maternal and Placental Blood Following Metal-on-Metal Arthroplasty Running Title: Maternal and Placental Metal Ion Levels

چکیده
Background: There has been a resurgence in the interest and use of metal-on-metal (MOM) bearing surfaces in hip arthroplasty. However, concerns remain regarding elevated metal levels in blood and urine and possible effect on the fetus of a pregnant female with a MOM bearing surface. Purpose: The purpose of this study is to compare metal concentrations in maternal and fetal serum in subjects with MOM hip arthroplasty to subjects without metal-containing implants. Methods: Maternal and umbilical cord blood was obtained from three patients with a MOM hip arthroplasty and seven patients without implants who served as controls. Maternal and placental serum metal levels were tested using inductively-coupled plasma-mass spectrometry. The serum concentrations of titanium, nickel, cobalt, and chromium were measured. Intergroup comparisons were made independent of the means with use of the Wilcoxon-Mann-Whitney test. Correlations were established with use of the Spearman rank-order correlation test. Results: Mothers with implants have statistically significant elevations in serum Cr and Co levels in comparison to control mothers. Babies from mothers with implants have statistically significant elevations in serum Co and Cr levels in comparison to control babies. Conclusions: The Cr and Co serum levels obtained from both mothers with implants and the offspring of mothers with implants are elevated in comparison to the control maternal and

Transplacental transfer of cobalt and chromium in patients with metal-on-metal hip arthroplasty: a controlled study.
Metal-on-metal bearings are being increasingly used in young patients. The potential adverse effects of systemic metal ion elevation are the subject of ongoing investigation. The purpose of this study was to investigate whether cobalt and chromium ions cross the placenta of pregnant women with a metal-on-metal hip resurfacing and reach the developing fetus. Whole blood levels were estimated usi...
متن کاملChromium and cobalt ion release following the Durom high carbon content, forged metal-on-metal surface replacement of the hip.
We evaluated the concentrations of chromium and cobalt ions in blood after metal-on-metal surface replacement arthroplasty using a wrought-forged, high carbon content chromium-cobalt alloy implant in 64 patients. At one year, mean whole blood ion levels were 1.61 microg/L (0.4 to 5.5) for chromium and 0.67 microg/L (0.23 to 2.09) for cobalt. The pre-operative ion levels, component size, female ...
متن کاملEvaluation of the method for analyzing chromium, cobalt and titanium ion levels in the blood following hip replacement with a metal-on-metal prosthesis.
High-resolution inductively coupled plasma mass spectrometry (HR-ICP-MS) can be used to measure metal ion levels in whole blood, although the accuracy and repeatability of this method is not yet known in the clinical setting. In this study, chromium, cobalt and titanium ion levels were measured in three whole blood samples, each collected at the same moment from 101 patients undergoing total hi...
متن کاملHip resurfacing versus metal-on-metal total hip arthroplasty - are metal ion levels different?
A systematic review and meta-analysis of trials comparing hip resurfacing to metal-on-metal total hip arthroplasty was conducted to determine if there is a difference in serum metal ion levels in patients receiving these implants. EMBASE and MEDLINE databases were searched from inception to December 2010 for all trials involving the use of these devices. Eligibility for inclusion in the review ...
متن کاملMid-Term Review of ADEPT Metal-On-Metal Hip Prosthesis. Functional, Radiological and Metal Ion Analysis
Background: There is much interest regarding metal-on-metal implants in medical and general media. Much of this has been regarding failure of specific implant systems and metal ion toxicity. We present our early mid-term experience of the ADEPT metal-on-metal system which has both modular and non-modular hip options. Methods: Functional assessment, blood metal ion quantification, and radiograph...
